All office visits are by appointment only. Every effort is made to see all patients on a timely basis. If you need to cancel or reschedule your appointment, please call our office immediately so that we may accommodate your needs and the needs of our other patients.
If you have had radiological testing done (x-rays, MRI, CT, Myelogram), please bring the CD’s and written reports with you to your appointment.
Please bring your insurance card(s) with you at the time of your appointment. If you participate in an HMO or Managed Care Health Plan, you must bring a referral for each office visit with your doctor. All co-pays are collected at the time of the visit.
A copy of the card(s) will be made and kept on file for billing purposes.
If your injury is work-related, verification of your Workers’ Compensation claim must be received by this office prior to your visit. Please include the following:
- Name, address and phone number of employer
- Name, phone and fax number of Workers’ Compensation carrier
- Claim Number
- Date of injury
- Attorney’s name and contact information
